As a free and confidential, centralized connection for resources, Hancock County’s Healthy365 and its team of health and wellness Support Navigators are here to help residents find solutions to our community’s most pressing health issues: addiction, mental health, chronic disease, obesity and the cost of healthcare by partnering for healing, improving health and wellness, alleviating suffering and delivering kindness one patient at a time.

We serve the communities of Greenfield, New Palestine, Cumberland, Fortville, McCordsville, Wilkinson, Shirley, and Spring Lake.

HISTORY OF HEALTHY365

Founded just five years ago, Hancock County’s Healthy365 is committed to making improvements in health, wellness and quality of life for our residents. By recognizing that the choices we make are often shaped by the choices we have available to us, our Support Navigators are dedicated to connecting residents to important local resources and providing overall wellness support – mind, body & spirit.

Our Healthy365 Connection Center conveniently provides access to countywide services and resources all in one place, and staff is able to connect individuals with the organizations that can help with their unique needs, from a food shortage to mental health or drug addiction, and so much more.

HOW HEALTHY365 WORKS

Before the launch of Healthy365, residents would need to call 211 for referrals and assistance navigating local health, human, and social service organizations. Now, doctors can refer a patient to Healthy365, or a resident can walk-in or call as a self-referral. Healthy365 works with all ages to ensure that whole health – physical, emotional, behavioral, and spiritual – is a top priority for the entire family. Our team of Support Navigators helps guide residents to the best place to receive the help they need. The team also is able to help make connections and assist with patient referrals.

IT’S TIME TO OPEN UP ABOUT MENTAL HEALTH AND SUBSTANCE ABUSE IN HANCOCK COUNTY.

Hancock County desperately needs professional services and support programs to address the growing mental illness and substance use problems in our community. To that end, our Hancock Health Foundation is raising $3.5 million to fund critical support and resources for our community.
